Gross or net: which should the share be on?
Net is more common where costs vary between doctors, because a doctor whose cases use expensive materials should not cost the clinic more. The calculator does both, per doctor.
Who carries the discount?
Set it per doctor: shared, so the share is on the discounted amount, or carried by the clinic, so the doctor is paid on the full price.
What is a guaranteed minimum?
A floor: the doctor receives the percentage or the minimum, whichever is higher. A fixed salary, by contrast, is paid on top of the percentage.
