ZATCA e-invoicing for clinics in Saudi Arabia

Saudi clinics do not have the option of invoicing now and complying later. Phase two means the invoice is cleared with the Zakat, Tax and Customs Authority as it is issued — so clearance has to happen inside the thing reception is already doing, not in a batch someone runs on Thursday.

Cleared as the invoice is raised

The clearance is part of issuing the invoice, not a step after it. Reception takes the payment and the compliant document is what comes out, with the QR code and the hash already on it.

  • Phase two clearance at the moment of issue
  • QR code carrying seller, VAT number, timestamp and totals
  • The tax breakdown on the same document as the treatment lines
  • Arabic and English on one invoice, not two versions of it
  • Nothing to export, upload or reconcile afterwards

The parts a clinic invoice actually has

A clinic invoice is rarely one service at one price. There is an insurance share, a patient share, a package being drawn down and sometimes a discount — and all of it has to survive the tax calculation intact.

  • Insurance share and patient share on the same invoice
  • Package and session draw-down as a line, not an adjustment
  • Discounts applied before tax, and shown
  • VAT per line where the rates differ
  • Credit notes and corrections cleared the same way as the original

Proof after the fact

Compliance is judged later, by someone who was not there. What matters then is that every issued document can be produced with its clearance status attached.

  • Clearance status stored against each invoice
  • Rejections surfaced immediately rather than found in a report
  • The archive held for the retention period without anyone maintaining it
  • Every reissue and correction linked to what it replaced
  • Exports for the accountant in the format they asked for

Who this is for

ZATCA is a compliance requirement, but it is reception who lives with it.

Reception

One action. The compliant invoice is the invoice.

Finance

Clearance status per document, and corrections that keep their link.

Owners

A position that can be produced on request rather than assembled.

Invoicing then submitting versus clearing as you issue

Both end with a compliant document. Only one of them ends with it today.

The way it is done now

  • A batch submitted later
  • A QR code added by a second tool
  • Insurance handled outside the invoice
  • Corrections tracked in a spreadsheet
  • Rejections found at month end

With Medicolize

  • Cleared at the moment of issue
  • The QR code generated with the invoice
  • Insurance share on the same document
  • Credit notes linked to what they replace
  • Rejections surfaced when they happen
Questions

Saudi Arabia — what clinics ask

Does Medicolize support ZATCA phase two?

Yes. Invoices are cleared as they are issued, carrying the QR code, the cryptographic stamp and the tax breakdown, rather than being generated first and submitted in a later batch.

Does the invoice work for insured patients?

Yes. The insurance share and the patient share sit on the same document, with any package draw-down and discount applied before the tax is calculated.

What happens if a clearance is rejected?

It is surfaced against the invoice straight away, with the reason, so it can be corrected while the patient is still a recent memory rather than at month end.

Can we issue invoices in Arabic?

Yes, and in both languages on the same document. The interface is right-to-left throughout, not a translated layer over a left-to-right layout.

Do we need a separate e-invoicing provider?

No. The clearance happens inside the invoice Medicolize issues, so there is no export, no second portal and no reconciliation between two systems.
