Medical report generator

How to use it

  1. Choose the purpose

    Each purpose shows the sections that reader expects. A report for an employer leaves the diagnosis out unless the patient asks for it.

  2. Fill in the sections

    History, examination, a table of investigations, diagnosis, treatment, current status, recommendations and how long any restrictions last.

  3. Set the letterhead and design

    Shared with the other tools. Choose a layout and colour, Arabic, English or bilingual, A4 or A5.

  4. Print or save the PDF

    A report longer than a page breaks between lines, and every further page carries the patient, the report number and the page count.

Write for the person who will read it

An insurer reads a report for the diagnosis, the treatment and what it cost; an employer reads it for whether the patient can work and for how long they cannot. Neither needs the other’s details, and an employer is rarely entitled to the diagnosis.

State what you found, what you did and what happens next, in short lines. Dates and results belong in the table, where they can be checked at a glance.

Save your design

Without saving, your clinic details, design and templates are cleared when the page reloads. Leave your email and they are kept in this browser from now on, and we will show you how they work in a Medicolize account, on every device and attached to patient files.

Is the medical report generator free?

Yes. No sign-up, no watermark and no limit on how many reports you print.

Does a report for an employer show the diagnosis?

Not unless you tick it. It shows fitness, restrictions and how long they last, and the diagnosis appears only when the patient asks for it to be included.

Can a report run to several pages?

Yes. The full letterhead prints on the first page, and every page after it carries a short header with the clinic, the patient, the report number and the page count.

Is patient information stored?

No. Everything about the patient stays in the page while you work and is gone when you close it. Only your letterhead and design are saved, and only if you choose.
