Write the referral
Urgency, the reason, history and findings in bullet points, what is enclosed, current medication and the question you want answered.
Print with the reply slip
Download PDF prints the letter and a detachable reply slip carrying your WhatsApp code, so the answer finds its way back.
What makes a referral get answered
A specialist reading a referral wants three things in the first lines: why the patient is here, how soon they need to be seen, and what you are asking. A letter that opens with the question gets an answer to that question; one that opens with a history gets a general opinion.
Say what you have already done and enclose it. Results that travel with the patient are tests nobody repeats, and a reply slip with your number on it is the easiest way for a busy colleague to close the loop.

Is the referral letter generator free?

What is the reply slip?
A second page the receiving doctor fills in by hand: when they saw the patient, findings, diagnosis, treatment, recommendations and who follows up. It carries your clinic name and a code that opens a WhatsApp chat with your number.
Are the receiving doctors saved?
Their name, specialty, facility and city are kept in a list once you press Save my design, in this browser only. Nothing about the patient is ever saved or sent.
What is the small code by the number?
It carries your clinic name, the document type, its number and its date, never anything about the patient, so whoever receives the letter can see where it came from.
