Medical certificate generator
Two short letters a clinic writes every day: proof that a patient attended, and whether they are fit for work, sport, school or travel. The Arabic wording follows the patient’s sex.
How to use it
Choose the certificate
Attendance, with the date, the times and who came with the patient, or fitness, with the date of the examination and the result.
Fill in the patient
Name, and the ID number if the certificate is going to an employer or a school. Attendance says medical review unless the patient asks for the diagnosis.
Set the result
Fit, fit with restrictions listed one per line, or not fit until a date. Nothing else is assumed for you.
Print on A5 or A4
Your letterhead, signature and stamp, a certificate number and a small code that shows where it came from.
Save your design
Without saving, your clinic details, design and templates are cleared when the page reloads. Leave your email and they are kept in this browser from now on, and we will show you how they work in a Medicolize account, on every device and attached to patient files.

Questions
Can it issue sick leave in Saudi Arabia?
No. In Saudi Arabia, sick leave for government bodies and large employers is issued through the official platform. These certificates are for private use, and a note saying so prints in the footer; you can switch it off outside Saudi Arabia.
Does the certificate state the diagnosis?
Not by default. An attendance certificate says the visit was a medical review, and the diagnosis is added only when you tick it at the patient’s request.
Is the certificate free?
Yes. No sign-up, no watermark and no limit.
Is anything about the patient saved?
No. The name, ID number and visit details stay in the page and are gone when you close it. Only the certificate number and your letterhead are kept, and only if you save.
