Choose the layout

Add the vaccines
Vaccine name, dose number, date and lot number, one row each. Common vaccine names are suggested as you type.
Sign and print
Save the PDF, or print it for the patient to keep with their documents.
What the travel certificate is, and is not
The travel certificate is a clinic letter confirming which vaccines were given, with the lot numbers a receiving doctor or employer may ask for.
It does not replace the International Certificate of Vaccination or Prophylaxis, the yellow card, which authorised centres issue where a country requires it, for example for yellow fever. The certificate says so on its face.

Is the vaccination record tool free?
Yes, with no sign-up and no watermark.
Can it replace the yellow fever certificate?
No. The International Certificate of Vaccination is issued by authorised centres. This tool makes a clinic record or letter.
Does it tell me which vaccines are due?
No. It records what you give and the next due date you enter. Schedules are the doctor’s decision.
Is the patient’s information stored?
No. Names, dates and passport numbers stay in the page while you work and are never sent to us.
