Discharge summary generator

The page a patient takes home after a procedure: what was done and under what anaesthesia, what to watch for, who to call, and when to come back.

How to use it

  1. Record the stay

Admission and discharge times, the procedure date, the surgeon and the anaesthesia.

  2. Describe the procedure

What was performed, the findings, implants or grafts with their count, medicines given during the stay and the number of the prescription handed over.

  3. Write what the patient needs

Instructions in brief, the warning signs that mean call us now, your emergency number and each follow-up date.

  4. Print and hand it over

Signed by the surgeon, numbered, and printed on your letterhead in Arabic, English or both.

Save your design

Without saving, your clinic details, design and templates are cleared when the page reloads. Leave your email and they are kept in this browser from now on, and we will show you how they work in a Medicolize account, on every device and attached to patient files.

The design and clinic details are kept in your own browser only. We receive your email and clinic name only: not the logo, not the signature and nothing about any patient.

Questions

Does it suggest medicines or doses?

No. It records what was given and refers to the prescription by its number. It suggests no medicines and no doses.

Where do the warning signs come from?

You write them. A button inserts a common list in the document’s language for you to edit, keep or delete.

Is the discharge summary generator free?

Yes. No sign-up, no watermark and no limit.

Is patient information stored?

No. The patient’s details stay in the page while you work and are gone when you close it. Only your letterhead and emergency number are saved, and only if you choose.

Procedures kept with the record
