A form, not legal advice
Each form follows the structure consent needs: what the procedure involves, the expected benefits, the risks, the alternatives, what happens if nothing is done, and the patient’s statement, with the doctor’s declaration beside the signature.
The wording is a starting point. Requirements differ by country and by procedure, so the form says on its face that it should be reviewed with your legal adviser before use.

Is the consent form generator free?
Yes, with no sign-up and no watermark.
Why are the risks empty?
Because they depend on your procedure, device and products. Write them as bullets and save the form as your version, or leave them empty and the headings print with lines to write on.
Is the on-screen signature saved?
No. It goes into the document you print or save as a PDF and is cleared when you close the page. Nothing about the patient is sent to us.
Is this form legally valid?
That depends on your country and the procedure. Use it as a structured draft and review it with your legal adviser before you rely on it.
