What is on the pages

How to use it
Add your letterhead
Put your logo, address, phone and licence number in the header of each letter.
Fill it from the visit
Copy the date and times from the appointment, not from memory.
Ask before writing a diagnosis
Leave the diagnosis out unless the patient asks for it, and tick the box when they do.
Sign, stamp and keep a copy
Give the original to the patient and keep a copy in their file.
Who it is for
Any clinic asked for proof of attendance by a school, a private employer or a gym, or for a fitness opinion before work or sport. The Arabic file runs right to left; the English file is the same two letters in English.

Can I use it as official sick leave in Saudi Arabia?
No. In Saudi Arabia, sick leave for government bodies and employers is issued through the official platform. This template is for private use.
Why is there no diagnosis by default?
An attendance certificate only needs to show the patient came. The diagnosis is the patient’s information, so it is added only when they ask.
What if the patient is fit with restrictions?
Tick that option and write the restrictions on the lines below, with a date to review them.
Who is responsible for the content?
The signing doctor. The template is a layout; what it says is the doctor’s statement.
