What is in the file

How to use it
Add your letterhead
Open the Word file and put your logo and clinic details in the header box.
Screen before the first session
Go through the contraindications with the patient and sign it off.
Write a line per session
Use the zone numbers from the map, and copy the lot number from the box for every injectable.
Log the photos
Record the views and the patient’s consent each time you photograph.

Questions
Why record lot numbers?
If a product is recalled or a patient reacts, the lot number tells you which patients received that batch. It takes seconds at the time and is hard to find later.
Can I change the zones?
Yes. The legend is plain text in the Word file; rename or renumber zones to match how your clinic describes areas.
Is photo consent the same as treatment consent?
No. The record logs photo consent separately for each set of photos. Treatment consent belongs on its own form; see the consent form pack.
Does it work for laser and injectables alike?
Yes. The settings column takes energy and spot size for a laser or units per zone for an injectable.
Sessions, units and photos on one chart
