The clinic front desk: a reception checklist from check-in to payment

The front desk is where the clinic either runs smoothly or falls apart. A receptionist who follows a consistent process means patients are checked in fast, the doctor's day stays on schedule, payments are collected, and the next appointment is booked before the patient leaves. A receptionist who improvises means all of those things happen some of the time.

This checklist is meant to be printed and kept at the desk. It covers the five phases of the day. Adapt the details to your clinic; keep the structure.

Opening (15 minutes before first appointment)

  • ☐ Open the clinic system. Review today's schedule for every doctor.
  • ☐ Flag any patient who has not confirmed. Send a WhatsApp reminder now.
  • ☐ Check for gaps in the schedule. Note them for walk-ins or waitlist calls.
  • ☐ Confirm each doctor's arrival time. If one is late, know by how much before the first patient arrives.
  • ☐ Check the waiting area: seating clean, water available, screen or signage working.
  • ☐ Cash float counted and recorded. Card terminal tested.
  • ☐ Printer has paper. Consent forms and intake forms stocked (if paper is still used).

Check-in (per patient)

  • ☐ Greet by name within 60 seconds of arrival. "Good morning, Mr. Hassan — you're here for Dr. Nour at 10:00."
  • ☐ Confirm the patient's phone number and ID on file are current.
  • ☐ New patient: confirm the pre-visit registration form was completed. If not, hand over a tablet or form now.
  • ☐ Insurance patient: check that the card is on file and valid. Note the copay or deductible due.
  • ☐ Tell the patient the expected wait. "The doctor is on time — about 5 minutes."
  • ☐ Mark the patient as arrived in the system. The doctor sees it.
  • ☐ If the patient is more than 15 minutes late, apply the late-arrival rule: inform them they will be seen after the next on-time patient, or offer to rebook.

During the visit

  • ☐ Watch the doctor's position versus schedule. If delay passes 15 minutes, inform the waiting patients.
  • ☐ Answer the phone within three rings. Use the booking script. Log every call that does not result in a booking.
  • ☐ Handle walk-ins: check availability, offer the next open slot or a gap in today's schedule.
  • ☐ Process lab results or referral letters that arrive. Attach to the patient record. Notify the doctor if urgent.
  • ☐ Prepare the next patient's file 5 minutes before their slot.

Checkout (per patient)

  • ☐ Receive the patient from the consultation room. Ask if the doctor requested a follow-up.
  • ☐ Book the follow-up now, before the patient leaves. Offer two specific times.
  • ☐ Generate the invoice from the doctor's notes. Confirm the amount with the patient.
  • ☐ Collect payment. Issue the receipt. Record the payment method.
  • ☐ Insurance patient: confirm the claim details are complete. Collect the copay.
  • ☐ If a prescription was issued, confirm the patient has it (printed or sent to their phone).
  • ☐ If a lab test or imaging was ordered, explain where to go and when results will be ready.
  • ☐ Thank the patient by name. "See you on the 14th, Mr. Hassan."

Closing (after last patient)

  • ☐ Reconcile the cash drawer. Count, compare to system total, record any difference.
  • ☐ Review every invoice issued today. Confirm all are paid or marked as outstanding with a reason.
  • ☐ Send tomorrow's appointment reminders.
  • ☐ Check tomorrow's schedule for unconfirmed patients. Flag them for a morning call.
  • ☐ Note any patient who did not show today. Mark as no-show in the system. Schedule a follow-up call.
  • ☐ Log any complaints or incidents. Escalate to the clinic manager if needed.
  • ☐ Lock cash in the safe. Shut down the terminal. Switch off the waiting area screen.

The three things most front desks skip

Booking the follow-up at checkout. Patients who leave without a follow-up booked return at half the rate of those who leave with one. The doctor says "come back in two weeks"; the receptionist books it that minute.

Telling the patient the wait. It takes five seconds and removes most of the frustration. An honest "about 15 minutes" is better than silence.

Reconciling cash daily. Clinics that count the drawer weekly find discrepancies they cannot explain. Clinics that count daily find them the same day.

What Medicolize does for the front desk

The checklist is easier when the system does half of it. Medicolize shows the receptionist today's schedule with confirmation status, flags late arrivals automatically, shows each doctor's live position, generates the invoice from the doctor's notes, and sends reminders on a schedule. The checkout screen prompts for the follow-up booking before the invoice can be closed. End-of-day reconciliation compares the recorded payments to the cash drawer total.

Print this

Print the five sections on one sheet. Laminate it. Keep it at the desk. Every new receptionist trains from it; every experienced one checks it on a busy day.

Medicolize is built around the front-desk workflow — check-in, scheduling, invoicing, reminders, and reconciliation in one screen. Request a demo.
